Short Term and Long Term Care 24 hours a day

All residents receive 24-hour routine and emergency medical care and are in constant interaction with our caring, skilled nursing department who provide continuous physical and emotional support while promoting mobility and flexibility. Our attending physicians, nurses, therapists, and social workers work together to provide every resident with an array of services customized for their needs, including restorative care, hospice/palliative care, psychiatry & psychological services, physical therapy, occupational therapy as well as speech therapy and dietician services.

Activities of Daily Living Care

Our certified nursing assistants are the backbone to the care we offer. Monuments CAN’s work with our residents to help them maintain their best possible quality of life. Nurse assistants help with dressing, grooming, showering, toileting, moving in and out of bed, bringing drinking water, reaching for a remote and most other activities of daily life. This team of compassionate assistants works their hardest to help our residents live comfortably.

Personalized Nutritional Services

Here at Monument, we take dietary service very seriously. Food is an essential part of everyday life. It nourishes and strengthens us, engages the senses and provides opportunities for socialization as friends gather for meals. Our menus include a wide variety of food choices including individualized diet plans to meet patients’ specific needs. Our staff dietician and managers do their best to ensure each resident has a diet that is catered for their optimal well-being.

Compliance

We are an imperfect nursing facility. There is a chance during a stay at our facility that we will make a mistake or miss an opportunity to do something the best way possible. In the event of a mishap, our Staff is committed to rectifying any situation in a timely manner. The ownership group has also partnered with Mednet Concepts to assist in investigations and to ensure serious concerns are addressed in an efficient and delicate manner.
