Family Counseling

The social work team works with families and residents to help them with their time in the facility. They are dedicated to helping each resident transition to the facility and to assist with any psychosocial needs. Each resident will receive individualized care plans designed to help achieve the best possible quality of life.

Dementia Care

At Monument, we have a dedicated unit to work with residents suffering from Dementia and Alzheimer’s. Our clinical team works hard to ensure each resident is constantly mentally and emotionally stimulated and assisted with living their best life in a safe environment.
